Chandler Hall is proud to announce the launching of the Palliative Care at Home Program!

The  accredited program is led by Medical Director, Dr. Maria J. Bertucci, MD. The program supports individuals who are living with serious illnesses, such as congestive heart failure (heart failure), COPD, Parkinson’s disease, Alzheimer’s disease and cancer. Patients will receive expert support from the Palliative Care team, including:

  • Guidance to provide relief from pain and uncomfortable symptoms.
  • Assistance to you and your family members in making medical decisions.
  • Coordination of care with other clinicians to help navigate the often complex health care system.
  • Help in making a plan to continue living well, based on your needs, preferences, values, beliefs, culture, concerns and goals.
  • Emotional and spiritual support from expert team members and community associations.

The team’s medical director, nurse practitioners, RN coordinators, social workers and chaplains are here for you. We see you as the “whole” person by supporting your unique medical, spiritual and emotional needs.
